In collaboration with the Michael G. DeGroote Innovation Initiative, the St. Joseph’s Healthcare Foundation hosted an innovation sprint in search of a solution to improve the caregiver experience for caregivers of CHF and COPD patients enrolled in the Integrated Comprehensive Care (ICC) program.
The team, consisting of Alvin Chin, Romi Lifshitz, and Shalev Lifshitz, created one of the two winning solutions for the innovation sprint. In the Loop, a modular dashboard for caregivers and other stakeholders in the ICC program, allows for identification, caregiver involvement, caregiver education, and metrics gathering for the ICC team to improve patient outcome and caregiver experience.
The team is now working with the St. Joseph’s Helathcare Foundation ICC Program to turn this innovation into a reality.
